Abstract

Childhood and adolescence are critical periods that affect adults’ health status. Therefore, the factors influencing the health of children and adolescents should be analyzed. In Korea, a wide range of youth-related health data has been obtained, both on the regional level and on the national level. This report summarizes the current status of studies related to the health of Korean children and adolescents. Data for which open access is offered include the Korea Youth Risk Behavior Web-based Study, the Panel Study on Korean Children, the Korean Youth Panel Survey, the Korean Children and Youth Panel Survey, and the Student Health Examination. In addition, the Health Examination of Korean Youth Outside of School, the Korean Children and Adolescents Obesity Cohort Study, the Korean Children’s Environmental Health Study, the Korea Youth Media Use and Harmful Environment Survey, the Comprehensive Survey of Korean Youth, and the Multicultural Adolescents Panel Study are summarized.

청소년시기는 성인의 건강상태에 영향을 미치는 중요한 시기이다. 따라서 청소년의 건강에 영향을 미치는 여러 가지 요소들에 대한 분석이 필요하다. 한국에서 청소년건강과 관련한 데이터는 지역 단위에서뿐 아니라 국가 단위에서도 수집되고 있다. 해당 데이터가 공개되고 있는 것으로는 청소년건강행태온라인조사, 한국아동패널, 한국청소년패널조사, 한국아동ㆍ청소년패널조사, 학교건강검사가 있다. 이외에도 학교 밖 청소년 건강검진조사, 소아비만 및 대사질환 코호트, 어린이 환경 보건 출생코호트, 청소년 매체이용 및 유해환경 실태조사, 청소년 종합실태조사, 다문화청소년패널조사를 요약하였다.

Abstract

OBJECTIVES
The field epidemiologist system of South Korea, which employs public health doctors who are relatively more readily available, was created in 1999 to ensure a ready supply of experts for epidemiological investigations and enable an effective response for new and reemerging infectious diseases. However, the 2015 outbreak of Middle East Respiratory Syndrome revealed limitations in the existing systems of management of field epidemiologists and communicable diseases.

METHODS

The present study aims to evaluate data on current states, administrative reports, and other literature on the field epidemiologist system that has been in place in South Korea for 16 years since 1999 and to suggest appropriate future improvements in this system.

RESULTS

By suggesting methods to evaluate the field epidemiologist system and training programs and by suggesting ways for the Korea Centers for Disease Control and Prevention to conduct evaluations on its own, the present study provides supporting evidence for improvement of systems for training of experts in epidemiological investigations. Moreover, based on the findings, this study also suggests methods to systematically train experts in communicable diseases management and a sustainable system to establish the basis of and develop strategies for a systematic and phased management of field epidemiologist training programs.

CONCLUSIONS

The present study suggests the possibility of establishing dedicated training facilities, revising the guidelines on training and improvement of the competency of public health experts, while not limiting the scope of application to communicable diseases.

목적 : 이 연구는 한국의 역학조사관제도의 바람직한 개선방향을 제시하고자 하였다. 방법 : 1999년 이후 16년간 한국에서 진행되어왔던 역학조사관 제도의 운영 현황자료, 각종 문헌, 행정보고서 등을 리뷰하고, 분석 및 평가하였다. 결과 : 연구 분석결과를 활용하여 보다 체계적인 감염병 관리 전문가 양성을 위한 방안과 역학조사관 교육 프로그램의 체계적인, 단계적인 운영 기반을 구축하고 전략 개발을 위한 지속가능한 수행체계를 제안하였으며, 역학조사관 제도 및 교육프로그램의 평가 방법을 제시하고, 향후 질병관리본부에서 자체적인 평가방향을 제안하였다. 또한 감염병에만 국한되지 않는 공중보건 전문인력 양성 및 역량 강화의 가이드라인 개정 및 전문 교육기관 설립에 대한 가능성을 제안함으로써 한국의 역학조사 전문인력 양성을 위한 제도개선을 위한 근거 자료를 마련하였다. 결론 : 이 연구에서 제안된 안을 바탕으로 향후 한국의 역학조사관제도의 적극적인 개선이 요구된다.

Abstract

OBJECTIVES
This study aimed to identify the characteristics of social networking service (SNS) users and to assess the relationship of those factors to user satisfaction with the SNSs of the National Cancer Information Center (NCIC) in South Korea.

METHODS

A Web-based survey was completed by 1,670 users of the NCIC SNSs, who provided data on the sources they consulted for information about cancer, determinants of SNS use, sources of NCIC SNS awareness, the time of day they used the SNS, and their satisfaction level.

RESULTS

Facebook users mainly became aware of the NCIC SNSs through an acquaintance’s recommendation, while Twitter was accessed through other SNSs or blogs. Users in their 30s were less satisfied with the NCIC SNSs than those in their teens and 20s. Browsing for cancer-related information on the Internet, prioritizing information quality, and engaging in active use were related to a high level of satisfaction with the NCIC SNSs. Individuals who were exposed to the NCIC SNSs through other SNSs or printed materials were less satisfied with their experience of the NCIC SNSs than those who received a recommendation from an acquaintance.

CONCLUSIONS

These findings may act as a catalyst to guide public health agencies to enhance their use of SNSs.

본 연구는 소셜네트워크서비스 사용자의 특성을 파악하고 우리나라 국가암정보센터 소셜네트워크서비스 사용자의 만족도에 영향을 주는 요인을 파악하고자 하였다. 국가암정보센터 소셜네트워크서비스 사용자 1,670명을 대상으로 암정보원, 서비스사용동기, 서비스를 알게된 경위, 사용시간대, 서비스 사용 만족도를 조사하였다. 분석결과, 페이스북은 주로 주변 아는 사람들의 권유로 사용하기 시작하였고, 트위터는 다른 사람의 소셜네트워크서비스를 통해 접근하고 있었다. 국가암정보센터에 대한 만족도에 영향을 주는 요인을 살펴보면, 10-20대에 비하여 30대의 만족도가 낮았으며, 인터넷을 통해 암정보를 얻고 있는 경우, 암정보의 질을 중시 여기는 경우, 활발하게 소셜네트워크서비스를 사용하고 있는 경우에 만족도가 높았다. 또한, 다른 사람들의 소셜네트워크서비스나 인쇄물을 통해 국가암정보센터 소셜네트워크서비스를 알게 된 경우 주변 사람들의 권고로 사용하게 된 경우에 비해 만족도가 낮았다. 공공보건의료기관들은 자 기관의 소셜네트워크 서비스 접근도 및 만족도 향상을 위하여 본 결과를 활용할 수 있을 것이다

Abstract

This study aims to provide a systematical introduction of age-period-cohort (APC) analysis to South Korean readers who are unfamiliar with this method (we provide an extended version of this study in Korean). As health data in South Korea has substantially accumulated, population-level studies that explore long-term trends of health status and health inequalities and identify macrosocial determinants of the trends are needed. Analyzing long-term trends requires to discern independent effects of age, period, and cohort using APC analysis. Most existing health and aging literature have used cross-sectional or short-term available panel data to identify age or period effects ignoring cohort effects. This under-use of APC analysis may be attributed to the identification (ID) problem caused by the perfect linear dependency across age, period, and cohort. This study explores recently developed three APC models to address the ID problem and adequately estimate the effects of A-P-C: intrinsic estimator-APC models for tabular age by period data; hierarchical cross-classified random effects models for repeated cross-sectional data; and hierarchical APC-growth curve models for accelerated longitudinal panel data. An analytic exemplar for each model was provided. APC analysis may contribute to identifying biological, historical, and socioeconomic determinants in long-term trends of health status and health inequalities as well as examining Korean’s aging trajectories and temporal trends of period and cohort effects. For designing effective health policies that improve Korean population’s health and reduce health inequalities, it is essential to understand independent effects of the three temporal factors by using the innovative APC models.

Abstract

OBJECTIVES
Goiter is a major public health problem, especially in developing countries such as Ethiopia. Hence, this study aimed to assess the prevalence and associated factors of goiter among children in Waghimra Zone, northeast Ethiopia.

METHODS

A cross-sectional study was conducted from April 8 to 25, 2015 in northeast Ethiopia. A multistage sampling method was used to select 454 schoolchildren. Data were collected using a pre-tested structured interviewer-administered questionnaire. Children were examined for the presence or absence of goiter based on the criteria of the United Nations Children’s Fund, International Council for the Control of Iodine Deficiency, and the World Health Organization. Salt samples from children’s homes were tested for iodine levels using a rapid iodized salt test kit. Data were entered into EpiInfo version 7 and exported to SPSS version 20 for analysis. Bivariate and multivariate logistic regression models were fitted, and adjusted odds ratio (aOR) with 95% confidence interval (CI) were computed to determine the level of significance.

RESULTS

The prevalence of goiter was 62.1% (95% CI, 57.5 to 66.5%). Being female (aOR, 3.09; 95% CI, 1.57 to 6.08), having a family history of goiter (aOR, 5.18; 95% CI, 2.43 to 11.03), and using non-iodized salt (aOR, 2.20; 95% CI, 1.12 to 4.38) were factors associated with goiter among schoolchildren.

CONCLUSIONS

The prevalence of goiter was high. Being female and having a family history of goiter increased the risk of goiter in children, but using iodized salt was protective. Therefore, we recommend ensuring universal access to iodized salt and increasing the awareness of the community of the importance of iodized salt utilization.

Abstract

OBJECTIVES
This study aimed to identify the factors affecting the cognitive function of elderly people in a community by gender.

METHODS

We obtained 4,878 secondary data of people aged ≥65 years in 2016 at a dementia prevention center in Gyeyang-gu, Incheon. Data were obtained through Mini-Mental Status Examination optimized for screening dementia and a questionnaire. The data were statistically analyzed using analysis of variance, analysis of covariance, and hierarchical regression.

RESULTS

There were significant differences in cognitive function according to gender, and the differences were significant even when age was controlled, but gender differences disappeared when education was controlled. Age, education, social activities, number of comorbid diseases, and alcohol drinking affected cognitive function through interaction with gender, but interaction with gender disappeared when education was controlled. Regression analysis showed that depression, cohabitant, social activities etc., had a significant impact on both men and women under controlled education and age. In men, the effect of social activities was greater than that of women, and hyperlipidemia had the effect only in women.

CONCLUSIONS

The differences in gender-related cognitive functions were due to differences in gender education period. The period of education is considered to have a great influence on cognitive function in relation to the economic level, occupation, and social activity.

연구결과 성별에 따른 인지기능의 차이는 성별 교육기간의 차이에서 기인된 것으로 확인되었다. 또한 연령, 교육, 사회활동, 동거인, 질병수, 음주가 성별과의 상호작용을 통해 인지기능에 영향을 미쳤으나 교육을 통제 하였을 때 성별과의 상호작용이 사라져 이들 변수들과 성별의 상호작용에 교육이 관련됨을 확인하였다. 회귀 분석결과 교육과 연령을 통제한 상태에서 우울, 동거인, 사회 활동 등 인지기능에 영향을 미치는 요인들이 남녀가 동일하였다.

Abstract

OBJECTIVES
The reported incidence rate of varicella infection in Jeju-do is higher compared with the national average. This study aimed to examine varicella vaccination history and evaluate clinical manifestation of varicella cases in Jeju-do.

METHODS

Based on the guideline suggested by the Korea Centers for Disease Control and Prevention (KCDC), two epidemic investigations for varicella infection were conducted in the first half of 2017. The history of varicella vaccination was confirmed using the Integrated Control System for Diseases and Health operated by the KCDC.

RESULTS

Out of a total of 60 elementary school children as the study subjects, all had been previously vaccinated against varicella. Twenty cases (33%) showed mild clinical manifestations and no complications.

CONCLUSIONS

As the government of Jeju-do has supplied a single-labeled vaccine since 2011, there is a need to evaluate the type of vaccination failure such as primary or secondary.

수두 발생 신고률에 있어 제주도는 전국에 비해 높은 수준을 보이고 있지만, 수둑접종률은 전국과 유사한 수준이다. 이에 2017년 상반기 수두 집단발생에 대응한 2건의 역학조사를 수행한 결과, 33%에서 돌발수두 임상 증상을 보였고 모두 백신 접종력이 있었다. 높은 발생 신고률은 지역사회 소아 개원의들의 높은 신고에 의한 것으로 유추할 수 있으며, 수두 백신 실패에 대한 추후 조사가 필요하다.

During the outbreak of the Middle East Respiratory Syndrome (MERS) in Korea in 2015, the Korea Centers for Disease Control and Prevention (KCDC) confirmed a case of MERS in a healthcare worker in Daejeon, South Korea. To verify the precise route of infection for the case, we conducted an in-depth epidemiological investigation in cooperation with the KCDC.

METHODS

We reviewed the MERS outbreak investigation report of the KCDC, and interviewed the healthcare worker who had recovered from MERS. Using the media interview data, we reaffirmed and supplemented the nature of the exposure.

RESULTS

The healthcare worker, a nurse, was infected while performing cardiopulmonary resuscitation (CPR) for a MERS patient in an isolation room. During the CPR which lasted for an hour, a large amount of body fluid was splashed. The nurse was presumed to have touched the mask to adjust its position during the CPR. She suggested that she was contaminated with the MERS patient’s body fluids by wiping away the sweat from her face during the CPR.

CONCLUSIONS

The possible routes of infection may include the following: respiratory invasion of aerosols contaminated with MERS-coronavirus (MERS-CoV) through a gap between the face and mask; mucosal exposure to sweat contaminated with MERS-CoV; and contamination during doffing of personal protective equipment. The MERS guidelines should reflect this case to decrease the risk of infection during CPR.

2015년 한국의 Middle East respiratory syndrome (MERS) 유행 기간 중 질병관리본부는 MERS 환자 심폐소생술(cardio–pulmonary resuscitation, CPR)에 참여한 간호사에서 MERS를 확진하였다. 이 사례에 대해 대전광역시 메르스 대응 민간역학조사지원단에서 심층역학조사를 실시한 결과 (1) CPR 수행 중 MERS-CoV에 오염된 에어로졸의 호흡기 침투, (2) MERS-CoV에 오염된 땀의 점막 침투, (3) 보호구 탈의 과정에서 MERS-Cov에 오염 등이 가능한 감염 경로로 파악되었다. 본 사례는 MERS 환자에 대한 CPR 수행 및 수행자의 보호구 착용과 관련하여 MERS 관리 지침의 개정이 필요함을 시사한다.

Abstract

OBJECTIVES
The purpose of this study was to establish a basis for improving or strengthening the preventive strategy against scrub typhus in Korea by comparing and analyzing the difference of prevention behaviors contributing to the occurrence of scrub typhus in Japan and Korea.

METHODS

The survey was carried out in Jeollabuk-do, which is a high risk and high incidence area, and Fukuoka Prefecture, which is a high risk and low incidence area. The study included 406 Korean farmers and 216 Japanese farmers. Data were collected through face-to-face surveys by interviewers who had completed standardized education.

RESULTS

Korean farmers have a higher percentage of agricultural working posture that involved contact with weeds than Japanese farmers (p<0.05). The frequency and proportion of weeding were lower in Korean farmers than in Japanese farmers (p<0.05). The level of knowledge about scrub typhus was significantly higher among Korean farmers than among Japanese farmers (p<0.05). Mostly, the behavior of agriculture work was more appropriate for Japanese farmers than for Korean farmers (p<0.05). The total average level of agricultural work was lower in Korea than in Japan, lower in men than women, and lower in part-time farmers than full-time farmers (p<0.05).

CONCLUSIONS

This study suggests that it is reasonable to develop and provide a program that can improve the level of preventive behavior taking into consideration the characteristics of the subject in order to reduce the incidence of diseases in high-risk areas for scrub typhus.

이 연구는 쯔쯔가무시증 발생 위험은 공통으로 존재하나 발생률 및 발생 건수에서는 현저한 차이를 나타내는 한·일 농업인들의 털진드기 매개체에 대한 접촉과 관련된 행태나 환경 관리의 차이를 파악하였다. 연구결과 한국은 일본에 비해 환경관리 수준이 낮고, 풀숲과 접촉할 수 있는 농작업 자세를 더 취하고 있었으며, 쯔쯔가무시증에 대한 지식 수준은 높으나 예방행태 수행 수준은 낮았다. 이 연구는 한국의 농업인들의 쯔쯔가무시증 발생을 억제하기 위해서는 예방행태를 향상시킬 수 있는 프로그램 개발과 제공이 필요함을 제시하고 있다.

Abstract

OBJECTIVES
The aim of this study was to investigate the clinical and demographic characteristics and some laboratory findings of hospitalized patients with acute opioid toxicity and rhabdomyolysis.

METHODS

This cross-sectional study investigated 354 patients hospitalized at Baharloo Hospital in Tehran in 2014 with acute illicit drug toxicity. Data were collected using an investigator-made checklist. The collected data (such as mortality rate, demographic data, and renal function tests, as well as serum biochemical findings) were analyzed by descriptive statistics and the chi-square test.

RESULTS

A total of 354 patients were admitted to the hospital in 2014 with acute illicit drug toxicity, including 291 males and 63 females. The total number of patients with rhabdomyolysis was 76 (21.5% of the total), of whom 69 (90.8%) were male and 7 (9.2%) were female. Most cases of rhabdomyolysis were associated with methadone abuse, followed by opium abuse. Rhabdomyolysis was most common in those 20-29 and 30-39 years old, with methadone and opium the most commonly abused illicit drugs. The mean blood urea level was 3.8±1.0 mg/dL, and the mean serum potassium and sodium levels were 3.8±0.3 mg/dL and 140.4±4.0 mg/dL, respectively. Five patients, all of whom were male, passed away due to severe renal failure (6.5%).

CONCLUSIONS

Toxicity caused by opioids is associated with clinical complications and laboratory disorders, such as electrolyte disorders, which can lead to lethal or life-threatening results in some cases. Abnormal laboratory test findings should be identified in patients with opioid toxicity in order to initiate efficient treatment.

Abstract

OBJECTIVES
To our knowledge, no previous study has systematically assessed the role of economic status in risky sexual behavior among people who inject drugs (PWID) in Iran. In this study, we used Blinder-Oaxaca (BO) decomposition to explore the contribution of economic status to inequality in unprotected sex among PWID in Tehran and to decompose it into its determinants.

METHODS

Behavioral surveys among PWID were conducted in Tehran, the capital city of Iran, from November 2016 to April 2017. We employed a cross-sectional design and snowball sampling methodology. We constructed the asset index (weighted by the first principal component analysis factor) using socioeconomic data and then divided the variable into 3 tertiles. We used the BO method to decompose the economic inequality in unprotected sex.

RESULTS

Of the 520 recruited individuals, 20 were missing data for variables used to define their economic status, and were therefore excluded from the analysis. Not having access to harm reduction programs was the largest factor contributing to the economic disparity in unprotected sex, accounting for 5.5 percentage points of the 21.4% discrepancy. Of the unadjusted total economic disparity in unprotected sex, 52% was unexplained by observable characteristics included in the regression model. The difference in the prevalence of unprotected sex between the high-income and low-income groups was 25%.

CONCLUSIONS

Increasing needle syringe program coverage and improving human immunodeficiency virus (HIV) knowledge are essential for efforts to eliminate inequalities in HIV risk behaviors among PWID.
