Persistence of vaccine-induced antibodies to A/H5N1 at 30 months and 36 months after vaccination in Vietnam

Abstract
OBJECTIVES
An A/H5N1 vaccine (IVACFLU-A/H5N1) was accepted for use in Vietnam; however, antibody persistence after vaccination has not been well characterized. We examined post-vaccination antibody persistence and related risk factors in individuals enrolled in the phase II IVACFLU-A/ H5N1 vaccine trial in Ninh Hoa, Vietnam, who received a 15-μg dose (2 injections 21 days apart).
METHODS
We used a longitudinal study design to follow 86 participants, without a control group. The participants tested as anti-A/H5N1 immunoglobulin G seronegative at baseline and received both doses of the vaccine. Blood was drawn at 30 months and 36 months after the complete vaccination to assess antibody status. Antibody persistence status was compared by demographic characteristics and exposure risk factors using univariate logistic regression.
RESULTS
In total, 84.9% and 52.3% of the population showed persistence of at least 1/10 of the A/H5N1 antibodies at 30 months and 36 months after IVACFLU-A/H5N1 vaccination, respectively. The odds of antibody persistence were higher in older people, but lower in people who had experienced flu-like symptoms in the past 18 months or between 2 visits. We recorded no differences between A/H5N1 antibody persistence and exposure risk factors, including having a poultry farm, coming into contact with poultry, and slaughtering and processing poultry.
CONCLUSIONS
This study demonstrated noteworthy antibody persistence, indicated by the seroconversion rate and geometric mean titer at 30 months and 36 months after the IVACFLU-A/H5N1 vaccine. Further studies should investigate older people and those who experienced flu-like symptoms to determine a suitable time for a booster shot.

The result can interpret that remarkable antibody persistence long after vaccination.

Abstract
OBJECTIVES
Cervical cancer is a major disease burden in Vietnam. This study aimed to estimate the incidence and mortality rates of cervical cancer in Vietnam (1999-2017) in comparison to those in Korea, where a population-based cancer registry and national cervical cancer screening program have been implemented.
METHODS
The estimated incidence and mortality of cervical cancer in Vietnam and Korea (1999-2017) were collected from Global Burden of Disease 2017 study. Estimated age-standardized rates (ASRs) in both countries were calculated utilizing the 1999-2017 population of each country and the World Health Organization standard population. The reported ASRs in Korea were also computed using data on incidence and mortality (1999-2017) and the Korean population from the Korea Statistical Information Service.
RESULTS
In Vietnam, the estimated incidence and mortality of cervical cancer decreased annually by 0.84% and 1.01%. In Korea, the trend of reported incidence showed a dramatic drop (1999-2007 annual percent change [APC], -4.53%) before stably declining (2007-2017 APC, -2.71%). Reported mortality also significantly decreased (2003-2008 APC, -6.63%), and then maintained a stable decline (2008-2017 APC, -3.78%). The incidence and mortality rates were higher in Vietnam than in Korea. The declining trend of incidence and mortality in Vietnam was slower than the corresponding trends in Korea.
CONCLUSIONS
A national screening program should be implemented for Vietnamese women aged over 30 to maintain, or even hasten, the decline in cervical cancer incidence and mortality. A population-based cancer registry may help monitor the effectiveness of a cervical cancer screening program.

국가 암 등록 체계와 국가 암 검진 프로그램이 아직 마련되지 않은 베트남에서 자궁경부암에 대한 질병부담을 줄일 수 있는 방향성을 제시하고자, 한국의 암 관리 현황과 질병부담을 비교한 연구입니다. 국가 암 등록자료를 구축하고 자궁경부암에 대한 국가 암 검진 프로그램을 도입한 한국과 베트남의 자궁경부암 발생률과 사망률 추이를 비교함으로 베트남에서도 체계적인 암 관리와 예방의 필요성을 강조하고자 한 연구입니다.

Abstract
OBJECTIVES
The prevalence of diabetes mellitus is rapidly increasing in Vietnam, particularly among adults aged over 45 years. This study estimated trends in diabetes and prediabetes prevalence and determined risk factors in Vietnamese adults (over 45 years).
METHODS
A cross-sectional study was conducted based on data from an annual diabetes screening program among people aged 45-69 years in an urban city in central Vietnam (Da Nang). Joinpoint regression analyses were performed to calculate the annual percentage change and p<sub>trend</sub>-values. Multinomial logistic regression analysis was used to determine risk factors.
RESULTS
In total, 3,765 men and 9,149 women were included in this analysis. The age-adjusted prevalence of diabetes and prediabetes in 2017 was 11.4% and 52.9%, respectively. The prevalence of diabetes was higher in men (15.1%) than in women (10.3%), but that of prediabetes was similar in both genders (53.4% vs. 52.8%). The prevalence of prediabetes significantly increased during the study period, whereas no upward or downward trend for diabetes was observed. The prevalence of obesity, abdominal obesity, hypertension, and dyslipidemia showed no obvious trend. Obesity, a high waist-to-hip ratio, hypertension, more severe abdominal obesity, and dyslipidemia were significantly associated with a higher risk of diabetes and prediabetes.
CONCLUSIONS
Diabetes and prediabetes were more prevalent among people aged over 45 years than in the general population. Da Nang has experienced a marked increase in the prevalence of prediabetes. These findings have significant implications regarding the need for nationwide public health interventions and management aiming at diabetes prevention and control.
